What is Pediatric Skilled Nursing (PSN)?

Pediatric Skilled Nursing (PSN) is a highly specialized area of healthcare dedicated to providing medically necessary, complex, and continuous care for infants, children, and adolescents (typically up to age 21) who have significant, chronic, or technology-dependent health conditions.

It is distinguished from typical childcare or basic care giving by the level of medical training and licensing required of the provider. PSN is delivered by Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) who possess specialized expertise in pediatric assessment and crisis management.

What does Pediatric Skilled Nursing (PSN) involve?

Pediatric Skilled Nursing focuses on managing the most medically fragile children and often involves:

  • Technology Management:
    • Ventilator and respiratory care for children with chronic respiratory failure.
    • Tracheostomy care including suctioning, site care, and emergency management.
    • Specialized tube feeding administration (G-tube, J-tube, NG-tube) and management of feeding pump equipment.
  • Complex Medical Conditions:
    • Management of severe seizure disorders requiring frequent medication administration and monitoring.
    • Care for children with neurological disorders (like severe Cerebral Palsy) or genetic disorders that impact multiple body systems.
    • Care for children with significant cardiac anomalies requiring continuous monitoring.
  • Procedures and Monitoring:
    • Complex wound care and dressing changes.
    • Administration of IV therapies (intravenous antibiotics, TPN, hydration).
    • Close vital sign monitoring and immediate, skilled response to acute medical changes.
  • Coordination of Care:
    • Collaboration with the child’s physician, specialists (e.g., pulmonologists, neurologists), and therapists (Physical, Occupational, Speech).
    • Training parents and family members on how to safely manage the child’s equipment and specific daily care tasks.
  • Care Settings: PSN is provided in the child’s home (most common), Pediatric Skilled Nursing Facilities (SNFs), or Prescribed Pediatric Extended Care (PPEC) Centers.
