At Elevate Care Providers, we offer a range of children’s healthcare services delivered at home, including pediatric nursing, general check-ups, developmental monitoring, post-hospital care, and other specialized care as needed. We work closely with parents, guardians, and (when applicable) the child’s physician to design a personalized care plan.

We cater to children of all ages who require medical care, developmental monitoring, or support at home. If your child has recently been discharged from hospital care, has a chronic condition, or needs follow-up care, we’re equipped to provide the necessary support — after a proper assessment and agreement on the care plan.

You can contact us via the “Make Appointment” or “Contact Us” page on our website. Once you reach out, we’ll guide you through the referral or intake process, finalize your child’s care plan, and schedule the home visit(s).

It depends on your child’s needs. For post-hospital care or specialized medical care, a referral from a physician may be required, we follow physician orders and coordinate with your child’s doctor for safe and effective care.

For general in-home support or pediatric monitoring, please contact us and we’ll advise based on your child’s situation.

We assess each child’s needs individually. If your child requires medical equipment, we can help coordinate delivery and setup, and train caregivers and family members on proper use, in close consultation with your physician.

Payment and billing terms depend on the services required and local regulations. We’ll provide you with all payment information, possible costs, and options during the intake process. Please feel free to ask us directly about payment mode when you contact us.

We understand that things can come up. Please contact us as soon as possible through our contact form or phone number to reschedule. We’ll do our best to accommodate changes, depending on availability and urgency of care.

All our care providers undergo rigorous vetting, including background checks, verification of credentials/licensing, and specialized pediatric training as appropriate. Safety, professionalism, and child-centered care are our top priorities; we ensure compliance with best practices and work closely with parents and physicians.
